Online Learning Space Reservation Fillup Form

"The LRC Online Learning Space Reservation Form is exclusive to NU students, faculty members,  and non-teaching personnel only. The submission of reservations for the regular term is at 7:30 AM to 6:30 PM and for the term break term, it is from 8:30 AM to 5:00 PM only, and it is only for Multimedia and e-Library reservations."

PRIVACY CONSENT: I understand and agree that by filling out this form, I am allowing the National University Learning Resource Center Philippines to collect, process, use, share, and disclose my personal information and also to store it as long as necessary for the fulfillment of the issue and concern, and in accordance with applicable laws, including the Data Privacy Act of 2012 and its Implementing Rules and Regulations, and the National University Learning Resource Center Privacy Policy.
